Service Description
Available for men! Initial consult with nurse practitioner for initial history and physical examination, review of symptoms, and blood work. Once results returned and reviewed, a plan is developed in regard to hormone replacement therapy to help improve these symptoms. Initial consult $420 follow up every 5 weeks $195 or every 10 weeks $360
Cancellation Policy
We value your time and ours, and to provide the best service to all our clients, we have established the following cancellation policy: 7 OR MORE DAYS BEFORE YOUR APPOINTMENT: - Any reschedules or cancellations must be made at least 7 days before your scheduled appointment to ensure a full refund of any prepayment or prevention of a cancellation fee. WITHIN 7 DAYS OF YOUR APPOINTMENT: - If you need to reschedule an appointment within 7 days of your appointment, we ask that you kindly call our center to reschedule your appointment within 7 days of your original appointment without penalty. - Any cancellations made within 72 hours of your booked appointment that are not rescheduled within 7 days will be charged a cancellation fee equal to the prepayment made to hold the appointment. Rescheduling within 7 days waives the cancellation fee if the new appointment is kept. NO SHOWS AND LAST MINUTE CANCELLATIONS: - In the unfortunate event of a no-show or if the appointment is not cancelled, a NO SHOW FEE of $100 will be charged to the credit card on file that is requested at initial visit. This charge is necessary to compensate for our time and to cover the resources allocated for your appointment.
